A) University Title Statement (General) – without personal particulars
甲) 大學正名聲明（一般） – 不包含個人資料

1. The Registry is pleased to provide a University Title Statement (General) – without personal particulars for graduates with CIHE certificates issued before 9 January 2024. A soft copy will be sent after you apply.
   二零二四年一月九日之前明愛專上學院的畢業生，註冊處將提供不包含個人資料的大學正名聲明。申請後，您將以電郵方式收到電子版副本。
2. The purpose of collecting personal data by means of this form is to process this application and may be transferred to other units within the University for necessary actions, where applicable.
   收集個人資料目的是處理是此申請，大學可能根據需要轉交本院內其他部門進行相關處理。
3. Without your expressed approval, or unless required by law, the personal data collected herein will not be disclosed to third parties other than those specified in Point 2.
   在未經您同意或法律要求的情況下，所收集的個人資料不會透露給第三方（第二點提到的部門除外）。

B) University Title Statement (Individual) – with personal particulars
乙) 大學正名聲明（個人） – 包含個人資料

1. Graduates with CIHE certificates issued before 9 January 2024 can apply for the University Title Statement (with personal particulars) with a fee of HK$150* per copy.
   二零二四年一月九日之前頒發的明愛專上學院證書畢業生，亦可以申請包含個人資料的大學正名聲明，每份收費為港幣一百五十元正*。
2. Application fee is non-refundable and non-transferrable. Online payment by Visa/Master/PPS is accepted.
   申請費用不可退款及不可轉讓，接受維薩/萬事達/繳費靈的網上支付。
3. The purpose of collecting personal data by means of this form is to process this application and may be transferred to other units within the University for necessary actions, where applicable.
   收集個人資料目的是處理是此申請，大學可能根據需要轉交本院內其他部門進行相關處理。
4. Without your expressed approval, or unless required by law, the personal data collected herein will not be disclosed to third parties other than those specified in Point 3.
   在未經您同意或法律要求的情況下，所收集的個人資料不會透露給第三方（第三點中提到的部門除外）。

1. Click the “Apply Now” button below to start the online application.

2. Select to apply the University Title Statement (General) or University Title Statement (Individual).
   選擇申請大學正名聲明（一般）或大學正名聲明（個人）。
3. Fill in your personal information.
   填寫您的個人信息。
4. Confirmation of information.
   確認信息。
5. Online payment of application fee of $150 per copy*, if applying for the University Title Statement (Individual).
   如申請大學正名聲明（個人），網上申請費用為每份港幣一百五十元正*。
6. Receipt of application with confirmation email.
   收到確認申請的電子郵件。
